If you've started researching hair transplants, you've probably come across three main technique names: FUE, DHI, and Sapphire FUE. Understanding the differences between them is important — but it's also simpler than most clinics make it sound.
Here's a straightforward comparison to help you understand which method might be right for you.
The Three Techniques at a Glance
| Feature | FUE | DHI | Sapphire FUE |
|---|---|---|---|
| Extraction method | Micro punch | Micro punch | Micro punch |
| Implantation method | Channel + forceps | Choi pen (direct) | Sapphire blade channel + forceps |
| Shaving required? | Yes (usually full) | Partial or no shaving possible | Yes (usually full) |
| Max grafts per session | 4,000–5,000+ | 2,500–3,500 | 4,000–5,000+ |
| Recovery time | 7–10 days | 7–10 days | 5–7 days |
| Scarring | Minimal | Minimal | Very minimal |
| Best for | Larger areas | Density, no-shave | Natural results, faster healing |
| Cost (Istanbul) | From £1,500 | From £2,000 | From £2,000 |
FUE (Follicular Unit Extraction)
FUE is the most established modern hair transplant technique and the foundation that DHI and Sapphire build upon.
How it works
Individual hair follicles are extracted from the donor area (usually the back of the head) using a micro punch tool, typically 0.6–0.9mm in diameter. The surgeon then creates small channels in the recipient area and places the extracted grafts into these channels using fine forceps.
Pros
- Well-established technique with decades of proven results
- Can transplant a high number of grafts in a single session
- No linear scarring (unlike the older FUT/strip method)
- Widely available — most experienced surgeons are trained in FUE
Cons
- Requires full head shaving in most cases
- Channel creation and graft placement are separate steps, adding procedure time
- Steel blades create slightly wider channels than sapphire
DHI (Direct Hair Implantation)
DHI uses a specialised tool called a Choi implanter pen that combines the channel creation and graft placement into a single step.
How it works
Grafts are extracted the same way as FUE. The difference is in the implantation: instead of creating channels first and then placing grafts, the Choi pen creates the channel and implants the graft simultaneously. The surgeon loads each graft into the pen and implants it directly.
Pros
- No need for channel pre-creation — potentially less trauma to the scalp
- Can achieve higher density in specific areas
- Partial or no-shave options available (ideal if you want to keep your current hair length)
- Grafts spend less time outside the body
Cons
- Generally limited to fewer grafts per session (2,500–3,500)
- Requires more surgical skill and experience
- Typically costs more than standard FUE
- Not ideal for very large coverage areas
Sapphire FUE
Sapphire FUE is an evolution of traditional FUE that uses blades made from sapphire crystal instead of steel to create the recipient channels.
How it works
The extraction process is identical to standard FUE. The key difference is in the channel creation: sapphire blades are smoother and harder than steel, allowing the surgeon to create smaller, more precise V-shaped incisions. This leads to less tissue damage and faster healing.
Pros
- Smaller, more precise channels = faster healing and less scarring
- V-shaped channels allow more natural graft angle and direction
- Less post-operative swelling and crusting
- Can handle high graft counts (4,000–5,000+)
- Sapphire blades maintain sharpness throughout the procedure
Cons
- Usually requires full head shaving
- Slightly higher cost than standard FUE
- Results depend heavily on the surgeon's skill — the tool alone doesn't guarantee quality
Our partner clinic in Istanbul offers all three techniques. The choice of method is discussed during your consultation and depends on your hair loss pattern, desired density, and personal preferences. The surgeon will recommend the best approach for your specific situation.
Which Method Should You Choose?
Choose FUE if:
- You need a large number of grafts (4,000+)
- You're comfortable with full head shaving
- You want a proven, cost-effective technique
Choose DHI if:
- You want to avoid shaving your head
- You need density enhancement in a specific area (like the hairline)
- Your hair loss is moderate (Norwood 2–4)
Choose Sapphire FUE if:
- You want the fastest possible recovery
- You want the most natural-looking results
- You need a high graft count with minimal scarring
The Most Important Factor
Here's what most articles won't tell you: the surgeon matters more than the technique.
A highly experienced surgeon using standard FUE will deliver better results than an inexperienced surgeon using the latest sapphire blades. Our partner surgeon has performed over 21,000 procedures across all three techniques. That depth of experience means they can recommend and execute the best approach for each individual patient.
The technique is a tool. The surgeon is the artist.
